Registration for 2021 Clinics

This year’s one-day clinics are from 5pm-7pm for children ages 10-14 on the day you choose below. Attend as many as you’d like! There are only 25 spots available for each date. If you have multiple children to register, you will need to refresh the page after each submission and register the next child on a separate form.

In consideration of my child being allowed to participate in the True Foundation Football Clinics, I, the parent/guardian, assume the risk of all injury and agree not to sue Brad Smith, Brad Smith’s True Foundation, the camp directors, coaches, assistant coaches, agents, or volunteers for any and all injuries caused by or resulting from participating in the Brad Smith True Foundation Football Clinics. By signing this waiver, I also authorize the use of pictures of the above-named participant to be posted on the True Foundation web site or advertising media published by Brad Smith’s True Foundation.
